Abstract

TUFM deficiency, associated with combined oxidative phosphorylation deficiency type 4 (COXPD4), is a rare autosomal recessive mitochondrial disorder caused by impaired intramitochondrial protein translation. This narrative review critically examined the provided corpus regarding molecular mechanisms, biochemical findings, clinical-radiological spectrum, and diagnostic implications of the disease. Available evidence indicates that dysfunction of mitochondrial elongation factor Tu disrupts the synthesis of mitochondrial DNA-encoded proteins and leads to combined respiratory-chain deficiency, hyperlactatemia, and marked neurological, cardiac, and multisystem vulnerability. A phenotype once regarded as almost exclusively infantile and fatal now includes intermediate forms and milder adult presentations. This expansion modifies prognostic interpretation and broadens the differential diagnosis of mitochondrial encephalopathies, leukodystrophies, and selected cardiomyopathies
