Abstract

TAF13-associated neurodevelopmental disorder, also known as autosomal recessive intellectual developmental disorder 60 (MRT60), is an ultra-rare TAF-opathy caused by biallelic hypomorphic variants in a core component of the TFIID complex. This narrative review synthesizes current knowledge on TAF13 molecular architecture, implicated pathogenic mechanisms, recognized phenotypic spectrum, and diagnostic implications. Available evidence supports a relatively stable clinical core comprising microcephaly, intellectual disability, growth retardation, and neurological abnormalities, to which the recent description of 46,XY disorder of sex development has been added. In parallel, structural and functional studies indicate that the disease arises from impaired TAF13-TAF11 heterodimerization, with downstream disruption of TFIID assembly and regulatory dynamics. MRT60 therefore provides a compelling model of how defects in general transcription factors may generate preferential neurodevelopmental vulnerability, while major uncertainties remain regarding natural history, full phenotypic breadth, and opportunities for molecular stratification
