Protocolos Diagnósticos e Terapêuticos Otimizados para Coinfecções em Adultos com HIV: Revisão Integrativa

This integrative review evaluated diagnostic and therapeutic protocols for coinfections in adults with HIV, outlining specific challenges related to bacterial, mycobacterial, fungal, parasitic, and emerging viral pathogens. Through systematic searches in PubMed, Embase, Scopus, and Lilacs (2015–2024), studies employing liquid culture, multiplex PCR, serologies, guided biopsies, and serum markers were selected, alongside optimized antiretroviral regimens, targeted antimicrobials, and immunomodulators. High mortality from unrecognized tuberculosis and malignant syphilis without prior screening was identified, as well as atypical presentations of Pneumocystis jirovecii, histoplasmosis, and parasitic overload. Superinfections with mpox, PVL-MRSA, and HDV underscored the need for multidisciplinary approaches. Immune Reconstitution Inflammatory Syndrome (IRIS) and severe adverse reactions demand precise monitoring and therapeutic adjustments. The study concludes that integrating broad screening, high-resolution diagnostics, and combined therapies, together with continuous surveillance, is essential to reduce diagnostic delays, personalize interventions, and improve survival and quality of life in people living with HIV.
